Terms and Conditions 

To find out more about the terms and conditions of the Eye Love Books program, please read the conditions below.

1. Schools must register to participate in the Eye Love Books program at www.opsm.com.au/prc before students can register their eye test.
2. To earn books, students must have an eye test at their local OPSM store between Monday,1^st February 2010 and Thursday 1^st April 2010 (Program Period) and then go to www.opsm.com.au/prc and complete their details including name, school, store and date where eye test was undertaken. Each student appointment will be verified by OPSM prior to being included in each school's appointment tally.
3. To earn books, a minimum of 10 eye tests per school must be completed within the Program Period.
4. A school's entitlement to books cannot be transferred or redeemed as cash.
5. Books will be selected by OPSM in its sole discretion from the NSW Premiers Reading Challenge booklist and will be provided to the school by 30th June 2010, unless otherwise advised by OPSM.
